In the January 3, 2010 issue of America, William Byron, SJ suggested the possibility of conducting exit interviews with Catholics who leave the Church.
Bishop David M. O’Connell of Trenton, N.J. took that suggestion seriously and at his request nearly 300 non-churchgoing Catholics in his diocese were surveyed.
The results are in many ways predictable but nonetheless instructive.
You'll find Byron's original article here and a report on the Trenton study, also in America, here.

At some point I hope the authors will write a follow-up article to relate the outreach efforts the bishop of Trenton made and what the results of those efforts were.
